PLACING AN ORDER: To place an order please call 1-800-458-5498 from 9:30 a.m. to

METHOD OF PAYMENT: Visa, Mastercard, American Express, Discover, and Check.

TERMS: Applicable sales tax added for shipments to Oklahoma residents. Oklahoma doctors and clinics are elligible for a sales tax exemption. Retruned check or C.O.D. will be charged a $20 reprocessing fee.

SHIPPING AND FREIGHT CHARGES: UPS shipping charges will be added to all orders, unless otherwise specified. Second day and next day shipments are available with an extra charge. Most orders are shipped within 24 hours. Shipping and handling fees vary according to location and weight. International orders will be charged appropriate shipping expenses, with customs, duties and brokerage payable by the customer at the destination.
Delivery options: UPS ground, overnight, 2-day air and Priority Mail.

RETURNS, EXCHANGES, AND CANCELLATIONS: Pre-approval is required on all returns and only merchandise sent freight pre-paid in new, unopened condition will be accepted. A 20% cancellation fee will be charged on all cancelled or returned items.

DAMAGE AND WARRANTIES: We guarantee every order to be in excellent condition at time of arrival.
Freight and Shipping damage is the responsibility of the carrier and must be noted before signing and accepting. Products and services are warranted by the manufacturer or provider of service and administered by their authorized service centers according to their published terms.
